Most individuals we serve are enrolled in an Ohio Medicaid waiver for people with developmental disabilities and have an active support plan.

Ask Us Directly

DD / IDD

Developmental Disability / Intellectual and Developmental Disability. An umbrella term for conditions that begin in childhood and affect cognitive, physical, or adaptive functioning.

Medicaid Waiver

A federal-state program that funds home and community-based services for individuals who would otherwise need institutional care. Most DD services are waiver-funded.

HPC

Homemaker/Personal Care — the primary service type Lumiven provides. Household task support and personal hygiene assistance.

ISP

Individual Service Plan. A personalized plan developed with the individual, their family, and their Support Coordinator that outlines goals, services, and authorized service hours.

Support Coordinator (SC)

A case manager assigned to individuals enrolled in the DD waiver system. They help coordinate services, connect families with providers, and monitor the ISP.

DSP

Direct Support Professional. A frontline care worker who provides hands-on support to individuals with DD in home or community settings.

The Process

What to Expect When You Reach Out

Here's what happens after you contact Lumiven Care, so there are no surprises.

Introductory Call

A Lumiven Care team member will call you back within 1 business day. We'll listen to your situation and answer any initial questions with no pressure.

Needs Assessment

We schedule a time to meet with your family (in person or by phone) to understand your loved one's needs, preferences, and current care plan.

Caregiver Match

We identify a caregiver who is the right fit, not just available. You'll have the opportunity to meet them before services begin.

Care Agreement

We complete the service agreement and required documentation together, coordinating with your Support Coordinator as needed.

Services Begin

Care starts on a schedule that works for your family. We check in regularly to ensure everything is going smoothly and adjust as needed.

Ongoing Communication

You'll always have a direct contact at Lumiven. We believe families should never feel like they're chasing their provider for answers.

FAQ

Questions Families Ask Us

Most individuals we serve are enrolled in a state Medicaid waiver program for people with developmental disabilities. If your loved one has an active case and a Support Coordinator, they may already be eligible. Contact us and we'll help you figure it out.
Consistency is important to us. We make every effort to schedule the same caregiver for each visit. In cases where a backup is needed, we communicate ahead of time and ensure the substitute caregiver is prepared.
Please tell us. We take all concerns seriously and will work with you to find a better match. A good caregiver-client relationship matters. We'd rather hear about a problem early than let it go unresolved.
For Medicaid-funded services, we bill Medicaid directly. Families typically have no out-of-pocket cost for covered services. We handle the administrative side so you can focus on your loved one's care.
Homemaker and Personal Care services are typically provided in the home. Community-based and accompanying services may be included depending on your loved one's Individual Service Plan and applicable waiver guidelines.

Common DD Services Glossary

The developmental disabilities system uses a lot of acronyms. Here's a plain-language guide to the most common ones.

DD / IDD
Developmental Disability / Intellectual and Developmental Disability. An umbrella term for conditions that begin in childhood and affect cognitive, physical, or adaptive functioning.
Medicaid Waiver
A federal-state program that allows states to fund home and community-based services for individuals who would otherwise need institutional care. Most DD services are waiver-funded.
HPC
Homemaker/Personal Care — the primary service type Lumiven provides. Includes household task support and personal hygiene and daily living assistance.
ISP
Individual Service Plan. A personalized plan developed with the individual, their family, and their Support Coordinator that outlines goals, services, and authorized service hours.
Support Coordinator (SC)
A case manager assigned to individuals enrolled in the DD waiver system. They help coordinate services, connect families with providers, and monitor the ISP.
DSP

Waiver Slot
A state authorization for an individual to receive waiver-funded services. Slots are limited. Individuals may be on a waiting list before receiving services.
DODD
Ohio Department of Developmental Disabilities — the state office that oversees DD services and certifies providers. Other states have equivalent agencies.
Person-Centered
A philosophy and practice of care planning that puts the individual's preferences, goals and voice at the center, rather than fitting them into a standardized program.
Respite Care
Temporary relief for primary caregivers (often family members). Allows family members to take a break while their loved one continues to receive appropriate support.
