- Employment-related information (experience, certifications, availability) submitted via the job application form.
- Staff account credentials (email and password) stored securely through Firebase Authentication.
- Shift scheduling and training completion records in the staff portal.
- Electronic Visit Verification (EVV) logs — including timestamps and device GPS coordinates — as required by Ohio law (ORC 5166.516) for Medicaid waiver billing.
We also collect limited technical data automatically, such as browser type, approximate location (country/region), and pages visited, through standard server logs and Firebase Analytics.
2. How We Use Your Information
- To respond to your inquiries and follow up on job applications.
- To manage staff scheduling, training compliance, and payroll administration.
- To meet Ohio DODD and Medicaid reporting requirements, including EVV submission.
- To improve our website and service delivery.
- To send operational communications to staff (account approvals, schedule changes).
We do not use your information for advertising or sell it to third parties.
3. Information Sharing
We share information only as necessary:
- Ohio DODD and Medicaid: EVV logs, training records, and service documentation as required for licensing and billing.
- Sandata Technologies: EVV data for Ohio Medicaid waiver billing once the integration is active.
- Payroll provider: Name, hours, and direct deposit information to process staff compensation.
- Firebase / Google: Our platform infrastructure for authentication and data storage. Google's data practices apply to data processed through Firebase.
- Law enforcement or regulators: When required by applicable law or court order.
We do not sell, rent, or trade personal information.
4. Cookies and Tracking
Our public website does not use advertising or marketing trackers. The staff portal uses Firebase Authentication session cookies solely to keep you logged in securely. These cookies are essential to portal functionality and cannot be disabled while using the portal.
5. HIPAA Notice
Lumiven Care provides Homemaker and Personal Care services under Medicaid waiver programs. Our public website and job application process do not collect Protected Health Information (PHI). Operational records related to service delivery are maintained in compliance with applicable state and federal requirements. Lumiven Care does not transmit or store clinical health records through this website.
6. Data Retention
We retain personal information for as long as necessary to fulfill the purposes described above or as required by Ohio DODD, Medicaid, and employment law (typically 7 years for service and payroll records). EVV logs are retained as required by ORC 5166.516.
7. Your Rights
You may request a copy of the personal information we hold about you, ask us to correct inaccurate data, or request deletion of your information (where not required by law) by contacting us at info@lumiven.care. We will respond within 30 days.
8. Children's Privacy
Our website and staff portal are not directed to children under 13. We do not knowingly collect information from children under 13.
